Telephonic support provided by the Broker Support Unit will be closed Monday January 21st in observation of Martin Luther King day and will reopen on Tuesday, January 22nd.

While Molina's telephonic support will be closed, their Enrollment Form Fax lines are open 24 hours, 7 days a week including holidays! Please fax your enrollment applications first thing in the morning by 12:00PM (MST) to be sure they are processed that day.

Guidelines require all Enrollment Forms be submitted to Molina within 2 days of receipt from the Medicare Beneficiary. They ask that you fax, as soon as possible, to insure Molina is in compliance with CMS Enrollment Form Processing Guidelines. 

Please reconcile your enrollment receipts by looking for the email from Broker Support Unit with the number of enrollments received. If you do not receive an e-mail, please contact the BSU right away. 

So, do not delay, Complete, Check and Fax those Enrollment Forms right away!
