Tax Forms 1095-B Distributed to Members
BlueCross BlueShield of South Carolina and BlueChoice HealthPlan members will begin receiving Internal Revenue Service (IRS) Form 1095-B from BlueCross and BlueChoice® .
We have begun mailing the forms to members who purchased individual coverage from BlueCross or BlueChoice. Members who received coverage from BlueCross or BlueChoice through their fully insured, employer-sponsored health plan will also receive a Form 1095. Here is a sample copy of the form.
Please note that we will not provide copies of the forms to agents or group leaders. Members will be able to download a copy of their Forms 1095-B by logging into My Health Toolkit® .
Form 1095-A Distribution
The Centers for Medicare and Medicaid (CMS) is currently mailing IRS Form 1095-A to members who purchased their 2015 coverage through the Federally Facilitated Marketplace (Marketplace). CMS will also post forms to consumers’ online Marketplace accounts. If you or your customers have questions about Tax Form 1095-A, please call the Marketplace directly at 800-318-2596.
