Assurity Offers New Options for Post-Issue E-Signatures

Assurity now accepts electronic signatures for several post-issue forms and requests via DocuSign and Adobe Sign. Members using e-signatures to submit documents must also submit the Certificate of Completion from DocuSign or the Final Audit Report Adobe Acrobat Sign page for the signature to be accepted.

The following post-issue forms and requests will be accepting e-signatures:

  • Policyholder Service Applications available through AssureLINK. State-specific applications for conversions, guaranteed insurability, policy change, and reinstatement.
  • Automatic Bank Payment for drafting premiums and/or loan repayments from a customer's bank account. Be sure to use the post-issue request form 18-051-005055.
  • Dividend Option Changes (Form 18-657-01155) to change the annual dividend on a traditional life policy.
  • Payment option changes (Form 18-656-05055).
  • Servicing Agent Change Request (Form 18-696-05055).

While these are currently the only post-issue service forms we accept e-signatures for, other information like address changes or distribution requests, like loans, are available for policyholders directly on the MyAssurity customer portal.
